Their sex is chosen by genes. Their sexuality if bisexual could become a choice of the individual when upon a more mature age. By socialogical conditioning, i mean that the days we did not care about what we wore, or who we chose to be, is a long time history, however, it is still true that what we wear and what name we call ourselves is irrespective of who the person truely is.

There are a number of bi names, for example I have an uncle and a female cousin both named Sydney.

Tom-boy works fine for either sex. But really, kids don't care.

If it was a situation that I was confronted with, I honestly have no idea what I would do, as a parent.

If you have a boy, at least in the US, you have to decide if you want genital mutilation or not. Aka circumcision. Personally I would strongly advise not.
