What does it mean to be possessed by something, and what is a malevolent entity?
Does possession mean replacing all of a person's brain patterns with that of another, or just part of the brain. How would such an action take place and who has the ability to undertake such an action?

To be inhabited by a malevolent entity can be interpreted in a wide sort of way as losing control of yourself. This means the previous regular person, the normal personality, surrendering to a new (and in this case "evil") personality. Usually it'd be in reaction of some sort of trauma or severe change in life I suppose. Metaphorically you can say the old person has been inhabited by a malevolent entity when something like this happens. It's likely that this is what most historical religious references refer to, even though they may present it as the spirit world due to the metaphor being considered real and being the only way they were capable of describing it. Exorcism could be as simple as therapy, religious or otherwise.
