Prolly mos of the people who have guessed in this thred have the tool that commonly replaces it... plus the replacment tool will work wit any type of screw/bolt head.!!!

I have a garage full of tools...Several tool cabinets...I must have spent over an hour, going thru drawers....I couldn't find anything like it.
 
OMG!!! :eek: It's a dual-purpose straight-blade and Phillips head screwdriver!

When the blades line up, it's a straight blade. When they're orthogonal, it's a Phillips head. :D

Duhh, I'm an idiot...I just went to the garage, I do have one...It's sitting on my work bench... Figured maybe there was a clue to your asking..I see no connection.
