If someone writes up a medical report for legal purposes and they directly quote you ie:

He referred again to the ‘good relationship’ he had enjoyed with Joe Blogs.

When in fact you said something along the lines of 'a very strong bond'. So should the quote 'good relationship' be a direct quote as at best the information has been paraphrased?

Paraphrases, generally, should not be placed in quotes.

However, you noted that this pertains to legal issues. I am not a lawyer in any state or country, and thus am not qualified to answer your question in those terms.
