I was raised in a family of 4, with a income of $9,000 annually, I worked my ass off, in the military saved and did what was ever necessary, I retired at 55, and I will have at a minimum $60,000 a year for life as my income, I didn't wait for the Government to had it to me, I don't have to rely on SS, I started working when I was 14, and that was on top of my farm chores that had to be done Morning and Night, season in season out, planting, and harvesting, the farm came first.
That's good... but how much was $9,000 worth back in the day when you were a kid? How much purchasing power did you have? I know it's a whole lot more then today, but how much would be equivalent in today's dollars exactly?

A letter is a letter....So it's somewhat ambiguous. It would be nice if they would use the decimal notation for acuity in studies such as this (20/20=1, 20/40=.5, etc) as the exact change in acuity would then be more clear....photodynamic therapy (PDT) has been out for a few years. It is promising, but I have not seen any results anywhere near as good as with the anti-VEGF treatments. Also, the medication needed for a single treatment cost like $1500, and multiple treatments are required.

An interesting side note, Avastin (the drug from which Lucentis is derived approved for use in colorectal cancer) is currently the most popular of the anti-VEGF drugs because a treatment is only $50 verses $1000 for the Lucentis! This is especially true for patients without secondary coverage to cover the 20% medicare does not pay.
Thanks again. I think you should push for you improved method of measuring improvement in vision. I bet there is a meeting in some place like Bermuda where you could do this and the IRS would help pay for the trip. ;) but from my POV,:bawl: The current ambigious system is crazy, but thanks again for explaining it.

Am I correct in my belief that once a drug is on the market FDA approved for X that doctor can LEGALLY give it for Y even if Y use has not been FDA approved?

I note the great economic pressure to use Avastin in the eye. I did not know it was only $50 a dose. It is obvious why Macagen fell into disfavor so quickly. I think a day or two ago I read that Avastin in a lung trial (probably Non Small Cell cancer, as that is the most common type of trials) did show some improvements, but none in life expectancy. Do not beleive FDA has acted yet on results - that is first "failed" for Avastin I know of.

Not true. Parents have nothing to do with your own personal decision to be a winner. ...
That is true - your decision is your decision, but if your parents are functionally illerate, there are no books in your school's "library" (only a few rats) essentially your only realistic chance of making that decision come true, at least until you go to jail, is dealing drugs or robbing the local stores (until you can get a car and hit the suburb homes).

I both believe you and respect you even though we often disagree. What you did when young did not require much in the way of "book learning." Those jobs, still exist, but not nearly enough for all that are only educated enough to do those "little formal education required" jobs.

Hell, you need at least a good highschool education to be allowed to operate the expensive machine that now digs ditches in the USA. (I do not think even the army will take you now without a highschool degree, but could be wrong on this.) I.e. there are not many jobs for the product (dare I call them a "graduate"?) of typical urban slum school in modern USA. (See my reply to Sanday on this.)

If you, as one of the "products" of a slum school, really do decide to be a "Winner" as Sandy suggests, then you should go into dealing drugs and use you brains to set someone else up to go to jail for you. Also an alternative to consider, as your supplier of drugs many kill you as well as the cops, Get an under age kid to rob houses by threating to kill or torture his little sister. That works well to here in Brazil if your education was not adequate for the legal jobs available.

The adequacy of your education by a "slum schools" (except in practical things like how to avoid rat bites) compared to the typical available job requirements is even less for "students" in many US urban schools, because the level of job technology is much higher than in Brazil. Here in Brazil, not only ditches, but large swimming pools are still dug by hand!

But that is were you start to make the decisions, even in the slum schools you can graduate, that means not Hanging and Banging on the street corner, stay away from the drugs, it means doing what ever is necessary to get that little piece of paper, and then if nothing else use the Military, it has provided the step up for millions to a better life, and you don't have to do a career, there are over a thousand different military job specialities, that are directly convertible to the civilian job market, I chose the Combat specialities because I could not stand to be stuck to a desk, and not be in the out doors.

One set of military job fields that translates directly to civilian employment is the medical fields, and today those jobs pay rather well.

The Computer fields do to, and I know that computer ability translate to $10 a Hr. + starting wages, hell even a old war dog like me, who didn't know a 1 from a 0 about computers, learned how to do maintenance on ATM's, reset them when they lost programing, re-cash them when they ran low on money, that started at $14.00 a hour.

So you have to start at school, and as Nike says, Just Do It!

There are winners and losers. The losers will make every excuse in the book. The winners will choose to win. There is no in-between. It's a decision.

I agree with you completely on this. The militrary, army included, is a great way out of a bad enviroment. Unfortunately, many young urban males with big ambitions and short time horizons see drug dealing as a quicker escape path. Some, especially tall blacks see basketball as their best hope, but few will ever make the big bucks they dream of. - In 99% of the cases, the military is their best ticket out.

Despite what Sandy thinks, ones attitudes are to a large extent are set before age 6 by your piers and parrents. Like you I was poor, a full need scholarship thur the Ph.D. level. I waited tables, washed dishes etc for my food. Repaired lab equipment and tutored for pocket money. One thanksgiving I did not have bus fare home. - I was too embarased to go eat in cheap lunch counter alone so decided not to eat at all that day and be truely thankful when the next day came. (Normally I ate two dinners in the fraternity houses where I worked in kitchen or waited tables, but they were closed and locked for the holiday.) I also tended bar at DEK parties for a couple of years, which in that era at Cornell, DEKs were the "drunkers of the hill."*

My father was an MD, but usually paid in home canned food or wild meat or chunk of smoked bacon or chicken and some eggs etc., not money, in his rural West Virginia practice so ate well and that was the only day I did not eat in my entire life. Unfortunately some Americans have quite a few of those days at the end of each month, including the elder ones who built American if their medical bills force a choice between eating and getting their medicine.

In some ways I might agree with Sandy about God being active. She thinks the drive to be a winner is entirly due to ones self and God. That it has nothing to do with the attitudes you absorb from your parrents and piers. Well, considering how rich and foolish America has been with that wealth, the poor schools US has available to many, etc. perhaps a just God is part of the reason why US is soon to collapse. ("soon" = in less than a decade).
-----
*I was washing dishes at DEK and did not know anything about mixing drinks. The DEK social manager came to the kitchen, talked to the cook and then me. He did not care about my lack of experience as bar tender. - "The brothers can mix their own. Just don't let them steal any cases" he said. I said OK, I can do that, but I will steal a jug of wine. He replied: "If that is is all I lose, I will be happy as a pig in mud" so we had a deal. (I kept my end of the bargin and half the guys on my dorm floor waited up for me to return most saturday nights. We drank that gallon in a few minutes and went to bed.)

yes the milatary is a good choice but not everyone is suited for that life.

if it were truely that simple everyone would be rich. how does someone get to be your age and still be that hopelessly naive.

yes the milatary is a good choice but not everyone is suited for that life.

So make your self suitable to the life, that is what it take to get ahead, you have to conform to the job to get ahead, with out the job you stay were you are.

Find the Pursuit of Happiness, with Will Smith, it is a true story, that man conformed himself to the job, you have to do what ever it take , long hours, and study hard, just do it.

No it isn't that simple, it's hard work, it's starting at the bottom, it's conforming to the job requirements, it working any job available, getting a work record, and living inside of your means, saving, driving used cars, no cigarettes, no weed, no beer if you can't afford it, hamburger instead of steak, the time will come for Beer, Steak, Fine Cigars, and Brandy, but you have to get their first.

I am still not rich, but I am well in the middle class, can pay my bills and take the wife out for Dinner every Friday night, buy Her a dozen roses, and take her to the Theater

Life is fine, and I am comfortable, and to get here wasn't a easy road, it was long and hard, and many time skimpy, but I did it.

so what are you saying if the poor black kid from the ghetto works really hard he will have the same chances as the white kid middle class kid from the suburbs?

No, it means that he has to work hard to get himself ahead, no excuses, no worrying about what some one else's is doing, You are responsible for your own actions, don't worry about someone else, worry about yourself, do what ever it takes to get out of the Ghetto, and make it legal, again if you want out you have to do it, that means making yourself fit the job, that means doing thing and jobs that you don't necessarily like, but when you work you get paid, and put 10% of that pay check into savings, Bonds, The Stock Market, even a passbook, if you can put $27,000 away by the time your
27, you can be a Millionaire when you retire, even if you never put another dime in, but if you keep saving, you can make it so that You and your Family will be set for life.

I worked hard every day of my life, I worked with what ever education I had at the time, I worked any job that was available, and I got every bit of school I could, I worked with a bad back, Broke once, two bad discs, I worked until I could retire, an did did it by 55, My military career, brought a retirement, and that was a job that was 24/7/365, weekends, Holidays, 72 hours on my feet, sleeping anywhere I could, doing what was required, going anywhere I was given orders for, that is how you get ahead, and after 20 years I received a retirement for doing what it took to be successful in the Military, but that retirement wasn't enough to live on alone, so I went back to work, as a Police Officer, Corrections Guard, Courier, Delivery Driver, I got my CDL (Commercial Driver License) drove Big Rigs, learned how to repair ATM, Became a Armed Courier, an scrimped and saved the whole time, and in the end I made it, hell I even was a maintenance man for McDonalds, what ever it took to keep money coming in, and I made it, retired at 55.

yes you did and that makes you one of the lucky few but to think just hard work will mean everyone will make it is naive. what if you live in an area with a higher cost of living, or if you saddled with high medical expenses or cannot afford to leave your home. there are many reasons why hard work alone will not get people ot of poverty. plus some people do not make enough to save. you keep on talking about how you made and thats all well and good but not every one can do it on hard work alone and i'm all for giving them helping hand that will allow them to reach their full potential.

And as long as you make excuses, and don't get out and just do it you will remain exactly were you are, bullheaded hard work will get you anywhere you want to go, maybe not as far as you want but it will get you off the bottom, the trip is worth it, and if you don't make it where you want to go just maybe you can give your kids enough help to get them there, I didn't get to be a millionaire yet, maybe never, but my son will be one, and hopefully my grandsons and daughters.

My Dad was a poor farmer, I made it to the middle class, my son will make it to the upper class,
JUST DO IT!!!

the jump from middle class to upper class isn't easy very few people get the chance your son if he is successful as you think he is will make upper middle class easy. and i am not making excuses hard work alone is not always enough to rise above poverty it is though almost always a to provide from you and your family but we talking about rising up out of poverty. i know what hard work can do my dad started out low as possible and att and managed to get as far as he possibly could without a degree. but the age of working hard rise above poverty is over. sucks but it is true to get a decent job to rise in economic class in this age you need a college or technical degree which the really poor cannot afford to get.
